Individuals enrolled in this program must qualify under the eligibility standards set by South Carolina’s Medicaid Home and Community-Based Services (HCBS) Waivers. By participating, they indicate their preference to receive care and support in their own homes or community settings rather than in institutional facilities.
Our Case Managers play an essential role in helping clients and their families coordinate and set up services with providers approved by Medicaid. The supports offered are based on each person’s assessed needs and may include, but are not limited to, the following:
- Personal Care (Level I & II) -Skilled Nursing Services
-Homemaker Services - Physical, Occupational & Speech Therapy
-Companion Care -Medication Management
-Respite Care -Home-Delivered Meals
-Behavioral Health Support -Nutritional Supplements
-Incontinence Supplies -Pest Control Services
-Transportation Assistance -Home & Environmental Modifications
-Adult Day Health Care -Care Coordination & Case Management
If you believe an individual may be eligible for South Carolina’s Medicaid Home and Community-Based Waiver programs, a referral can be made for further support and evaluation.
Referrals may be submitted by anyone familiar with the person’s situation, with their consent. All applicants must meet the eligibility requirements for South Carolina’s Home and Community-Based Services (HCBS) Waiver programs.
